COVID Contact Form

If your child develops symptoms of Covid-19 on Saturday 29th May or Sunday 30th May you must complete the form below.


If your child develops symptoms you must arrange for them to have a test. Once you have received the results of this test you must complete the online form on our website again to let us know the outcome. You must let us know whether the test is positive or negative. This will allow us to communicate with the DfE and Public Health England to undertake our contact tracing responsibilities. Please do not email any of the school email addresses directly as these will not be monitored during the half term.


If your child develops symptoms from Monday 31st May, the school does not need to be informed. Parents and carers should follow contact tracing instructions provided by NHS Test and Trace.


The main symptoms of coronavirus are:


  • high temperature – this means you feel hot to touch on your chest or back (you do not need to measure your temperature)
  • new, continuous cough – this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours (if you usually have a cough, it may be worse than usual)
  • loss or change to your sense of smell or taste – this means you’ve noticed you cannot smell or taste anything, or things smell or taste different to normal
